Acetyl-L-carnitine transports fatty acids into mitochondria for energy production and may help support nerve regeneration. It has the strongest evidence for diabetic and chemotherapy-induced peripheral neuropathy with more modest evidence for cognitive support.
Form: Acetyl-L-carnitine hydrochloride
Typical dose studied: 500–2,000 mg/day in divided doses
Timing: Morning or early afternoon with or without food
ALCAR crosses the blood-brain barrier more readily than L-carnitine. May cause mild GI upset in some individuals.

Avoid with: Epilepsy / Seizure disorder; Hypothyroidism / Hashimoto's thyroiditis
Interacts with: Warfarin — rare interaction reports; Thyroid hormone replacement — consult clinician
